I hereby enroll my child in the school instrumental music program. Learning an instrument is a commitment in which I will support by encouraging my child to attend each week and establishing a regular practice routine at home. I understand that there are no refunds after the third week of class or if a class is cancelled due to circumstances beyond The Orchestra Place's control. A minimum number of students is required to hold a class and if a particular class is cancelled due to low enrollment, the student will be issued a full refund.

I understand that regular attendance is very important and that three or more unexcused absences may result in my child being dropped from the program.

I also hereby recognize the importance of having a good, quality, working instrument and understand that my child must have an instrument brand name listed on the approved brands list in order to participate. Inferior quality instruments are not acceptable for use in the music program.

I also hereby grant to The Orchestra Place, its sponsors, organizers, and/or assigns The Orchestra Place the irrevocable and unrestricted right to use and publish photographs (including video photography and performance) of my child, or in which my child may be included, for publication, electronic reproductions (websites) and/or promotional materials or any other purpose and in any manner or medium. In addition, I grant my permission to alter the same without restriction; and to copyright the same. I hereby release the photographer and The Orchestra Place from all claims and liability relating to said photographs.
